Human responsibility and oversight
Keep a real decision pointHuman oversight matters only when a person has enough time, information, authority, competence, and independence to question the system. A person clicking approve on hundreds of outputs is a rubber stamp, not meaningful oversight.
Before an AI-generated benefits decision reaches a person, the reviewer sees the evidence, known limitations, affected rule, confidence limits, alternative outcome, and a clear way to pause or escalate.
Do not use ‘human in the loop’ as a blanket claim of safety. Automation bias, workload, unclear ownership, and incentives can make nominal review ineffective.
Name the accountable owner, review trigger, evidence shown, time available, override power, appeal path, and record retained.
